Job Responsibilities:
1.Responsible for completing all MDS, admission, significant change, discharge, re-entry tracking forms quarterly and annual assessments. Responsible for the completion of all Resident Assessment Protocol and entering all necessary information into the computer system. Maintains records of all MDS/RAPS for a 15-month period. 2.Ensures all departments involved in the Resident Assessment Instrument (RAI) process complete their MDS sections/RAPs and Care Plans in according to the time line set by the MDS Coordinator to stay within state guidelines. Prepares and distributes time lines and calendars to all departments involved in the RAI. 3.The MDS Coordinator must schedule in conduct care plan meetings for all residents. Sends out letter to all residents’ families for notification of care plan meetings and schedules according to their availability. 4. MDS Coordinator successfully completes a preliminary care plan on all new admissions within in 72 hours of admission. Responsible for completing comprehensive patient specific care plans on all residents every 90 days. Updates resident care plans as needed with necessary changes. Backs up computer system weekly to ensure proper transactions. 5.Coordinates all departments involved in the care plan process complete patient specific care plans in a timely manner and in accordance with the regulations set by the Division of Aging. 6.Archives all MDS assessments and transmits them successfully to the Division of Aging. Minimal of every 30 days - preferably weekly. 7.Notifies supervisors and charge nurses of any concerns, problems or updates professional and in a timely fashion. 8.The MDS Coordinator must conduct chart audits on all admissions and re-admissions for accuracy, competency and ensures that entire admission/re-admission process was completed as outlined by nursing policies. Audits all physician telephone orders for accuracy and ensure that they are completed properly and timely. 9.Monitors and tracts all facility infections. Ensures that infection control forms have been completed on all residents on antibiotics. Follows up on all resident cultures and input all diagnosed infections in to the computer system. Compiles monthly, quarterly and yearly infection control reports. Computes infection % per every 100 residents monthly. 10.Works with physicians, team members, other departments to ensure that they are aware of the completeness and accuracy of the assessments. Coordinates written plan.
